KEPT INC is Hiring a Financial and Community Connections Account and Care Manager Near Louisville, KY

Kept, Inc is seeking a compassionate, self motivated, team player! We are excited to expand our Financial and Community Connections program to serve more people in Kentuckiana! At Kept, Inc, we help vulnerable adults avoid homelessness, exploitation and abuse and improve their overall safety and wellness through care coordination and representative payee services.

Our account managers handle a wide array of tasks on behalf of the individuals we serve. Each one has a group of individuals they work directly with to help them manage their finances and daily affairs. This is an office-based position. Tasks include:

  • Assist clients in planning resource use and assuring protection of resources
  • Complete budget development with client/care team, initially and ongoing
  • Complete SSA accounting reports and disability review reports and questionaries on behalf of the individuals we serve
  • Ensure all bills are identified and prioritized for payment initially and ongoing
  • Balance wants with needs (housing, food, current medical expenses first), based on regulatory requirements
  • Complete forms and documentation using electronic filing system (google suite and case management software)
  • Manage multiple accounts using fiduciary software
  • Complete annual updates and annual financial resource and benefit reassessment
  • Ensure client remains eligible for all social benefits- this may include completing annual or bi annual reporting or renewal processes (transaction reports)
  • Ongoing assessments to identify other services or benefits client may need or be eligible for.

Coordinate receipt of those services or benefits which client is eligible but does not already have. This may require an internal referral to Care Coordinator.

  • Make timely updates of the budget and other relevant documents
  • Respond to all inquiries by client, guardian, care team or other agencies within 24 hrs using google voice app, email and text messages.
  • Monitor for client changes and report relevant changes to SSA and team members
  • Document all work in case management software
  • Consult supervisor on asset predictions, planning and monitoring
  • Complete quarterly client welfare checks (includes annual review) via video chat, phone call; text/email conversation or in person visit to review budget and complete needs assessment.
  • Work effectively with community partners and client team members
  • Help create person-centered service teams and partnerships for those we serve

Minimum Requirements:

High school diploma or GED and some experience in the health or human services.

Have a state police criminal record check for any state resided in during the previous year. If there is a prior conviction of any offense delineated in KRS Chapter 17.165

(1)-(3) or prior felony conviction, the applicant shall not be employed. Thorough knowledge of community-based services/supports, community resources and funding sources, quality of services, and social and health intervention techniques is preferred.

Preferred ability to facilitate the cooperation of individuals served, their families, service providers, paid staff, and volunteers and stimulate the cooperation of community agencies or individuals. Possess the ability to speak, write, and problem-solve and the ability to maintain records in a clear and concise manner. Possess the ability to handle multiple tasks, be computer literate, possess a required valid state driver’s license, insurance, and reliable transportation.
