WSH has a 168-year history in the Hopkinsville community. It is a state acute adult psychiatric facility that provides inpatient psychiatric evaluation and treatment for adults from 34 counties of Western Kentucky. WSH is on the National Register of Historic Places and is accredited by the Joint Commission. We are seeking a Social Service Worker I to join our team.
This job requires mandatory COVID 19 vaccination or an approved exemption in accordance with CMS-3415-IFC from the Centers for Medicare & Medicaid Services. This means if appointed to the position, the applicant must have completed the primary vaccination series for COVID-19, which means the administration of a single-dose vaccine or the administration of all required doses of a multi-dose vaccine upon commencement of employment. The vaccine received must be one of the FDA-approved or FDA-authorized vaccines. Proof of vaccination will be required. If a medical or religious exemption is sought, the candidate must submit paperwork to the employer prior to appointment for review and approval.
Responsibilities include but are not limited to:

• Assesses patients' and families' needs, provides counseling to patients and families, serves on multidisciplinary treatment team and develops goals, plans and interventions.
• Coordinates patient discharges with other disciplines and makes referrals to community agencies.
• Screens referrals to the hospital and interprets agency mission and admission criteria to the public.
• Maintains current and confidential case records.
• Serves on agency committees' work teams.
• Other duties as assigned.
