Title: Manager, Cardiac Rehabilitation Program

Job Overview: The Manager of Cardiac Rehabilitation is a Clinical exercise Physiologist who oversees and enhances our cardiac rehabilitation program and delivers patient care. The role demands a unique combination of clinical expertise, leadership skills, and a passion for patient-centered care. 

Key Responsibilities:

• Direct Patient Care (Approximately 2/3 of Role):
  • Conduct individual assessments, develop personalized rehabilitation plans, and provide ongoing patient support.
  • Monitor patient progress, adjusting plans as necessary for optimal recovery.
  • Offer expertise in cardiovascular disease management, including lifestyle and behavioral coaching.
  • Collaborate closely with healthcare professionals to deliver integrated care.
• Program Management (Approximately 1/3 of Role):
  • Oversee program operations, including staff management, resource allocation, and compliance with standards like CMS/AACVPR
  • Develop and implement strategies to enhance program quality and patient satisfaction.
  • Analyze program data to identify trends, improve outcomes, and drive strategic decisions.
  • Ensure the program stays current with the latest in cardiac rehabilitation practices and technologies.
  • As the team grows, the management portion is expected to increase over time. 

Qualifications:

• Master's degree in a related field preferred; Bachelor's degree required.
• Clinical exercise physiology certification (ACSM) or the ability to obtain it within one year.
• Experience in managing cardiac rehabilitation programs.
• Experience in staff training, education, and competency development in a clinical setting.
• Demonstrated ability to deliver compassionate, patient-centered care.
• Experience with virtual care delivery is a plus.
• Certifications in diabetes education, wellness coaching, or other relevant areas are highly valued.
