Position Summary:
The Nurse Case Manager/Delegating Nurse works with and supports an interdisciplinary team to support the medical wellbeing of persons served. The Nurse Case Manager/Delegating Nurse trains direct support professionals on medication administration and nursing skills, in accordance with Maryland Board of Nursing, Developmental Disabilities Administration/COMAR regulations. The Nurse Case Manager/Delegating Nurse supervises direct support professionals in the implementation of the medical components of individual service plans.
Essential Responsibilities:
· Support direct support professionals to ensure that persons served, on assigned caseload receive appropriate medical services (medical, dental and psychiatric appointments; lab and diagnostic studies; medical and dental procedures, etc.) in a timely manner; including immunizations remain current
· Communicate and provide training on procedures and treatments to persons served and direct support professionals
· Delegate nursing functions to unlicensed direct support professionals; supervising the administration of medication and nursing tasks for assigned caseload
· Provide person specific training to unlicensed direct support professionals as outlined in a person’s individual plan
· Complete all required assessments and reviews; including 45-day reviews for persons served that have delegated nursing functions, for assigned caseload in a timely manner
· Develop, revise and monitor nursing diagnosis, nursing care plans and nursing goals for assigned caseload
· Ensure direct support professionals are following procedures for physician visits, emergency coverage, records, consultants, and rehabilitation services for assigned caseload
· Provide adequate supervision of all CMT’s working under the RN’s Delegation, to assure proficiency and competency, to safely administer medications
· Maintain all health and nursing related documentation in a timely manner
· Provide On-Call service for assigned caseload and will respond to on-call phone calls and text messages in a timely manner and provide sound nursing advice; will also participate in an On-Call rotation with the other RN’s on the Nursing Team
· Assist the Health Services team with unassigned routine duties (taking calls for other Nurses, providing services to individuals that are not part of assigned caseload, vaccine clinics, assisting with pharmacy deliveries, etc.)
· Assist and advise both the Day/Vocational Director, Residential Director, and Program Coordinators in meeting the health needs of persons served
Minimum Qualifications:
· Must hold a State of Maryland RN License from the Maryland Board of Nursing
· Must hold current CPR Certification
Education:
· Graduate from an Accredited school of Nursing
Experience:
· Minimum of one year plus in providing nursing services for people with intellectual/developmental disabilities, or related disabilities
Physical requirements:
· Ability to lift a minimum of 50 pounds
· Ability to lift and transfer individuals
License/Certifications:
· Mandt, Certification as a Delegating Nurse Trainer for the MTTP, DDA training, Bloodborne pathogens, and CPR/First Aid certifications required. Training to obtain certifications will be provided.
Knowledge, Skills, and Abilities:
· Excellent oral and written communications; strong interpersonal skills
· Proficient computer skills
· Proficiency in MS office package required; knowledge with database and electronic medical records preferred
· Ability to work independently, be flexible and productive in a fast-paced environment and ability to manage a multidimensional workload
· Skilled in managing relationships; able to communicate effectively with people with disabilities, staff, and families; liaise with colleagues at all levels; able to work internally with staff across departments

Pay: $35,000.00 - $45,000.00 per year
