As a member of our team you will work with adults with mental illness in the development & implementation of a service plan that provides clients with access to and utilization of appropriate community resources.

Are you a problem solver who will provide support and assistance in the community? You will assist with problem solving, decision making and coping skills to maximize strengths and eliminate barriers for our consumers. The adult case manager helps consumers in getting basic needs met such as housing, food and entitlements. You will assess and evaluate, set goals, and develop a comprehensive individualized support plan describing the services or interventions needed. Requirements: Certification as a Mental Health Rehabilitation Technician - Community (MHRT/C) or a Provisional MHRT/C required, and experience in the field is preferred. Valid driver's license.
