Chronic Care Coordinator
Must be located near Waukegan IL area as employee will report to job site. This job requires a nursing or medical assistant license (CMT, CNA, or LPN), excellent communication skills and computer/electronic health records experience is a plus. This job includes phone patient contact and it's goals are providing support for the Physicians, Care Management program by coordinating a comprehensive plan of care for the patients. The job will require facilitating the transmission of information between providers and patients, as well as access to services needed and referral services to the patients and their families.

Pay: $13.00 - $16.00 per hour
