Nurse Case Manager w/ prior Skilled Nursing Facility Experience

Wage:

  • $40-$47/hr LVN
  • $48-$55/hr RN

Kearny Mesa Convalescent, a Generations Healthcare is in search of a Nurse Case Manager to join our team.

Job Description:

The primary purpose of the Nurse Case Manager is to develop, evaluate, implement health care plans for individual patients, manage cost effective medical plans, while providing the highest level of patient care. The Case Manager also communicates treatment plans between families, doctors, providers and patients.

Essential Duties (include but are not limited to):

Evaluate and update patient health care plansAssessment for new patientsReview test results from medical examsImplement Physician orders and follow up with treatment plansCommunicate with families, health providers and patientsDocumentation of medical recordsProvide emotional support to families and patientsProvide education about health care plans to patients and families

Qualifications:

Must possess, as a minimum, a Nursing Degree from an accredited college or university.
  • Proof of COVID-19 Vaccination will be required for all facility employees as of September 30, 2021 (requests for accommodation due to disability or religious belief will be reviewed).
Must have, as a minimum, one (1) years of experience as a supervisor in a hospital, nursing care facility, or other related health care facility.Must have training in rehabilitative and restorative nursing practices.Strong communication, excellent customer service, teamwork and organizational skillsMust possess a current, unencumbered, active license to practice as an RN in this state.Must demonstrate the knowledge and skills necessary to provide care appropriate to the age-related needs of the residents served.Be a supportive team member, contribute to and be an example of team work and team concept.Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the general public.Must be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to nursing care facilities.
