The Job:
We are currently recruiting for an entry-level Community Support Specialist to join a dynamic team that serves underprivileged communities. The Care Coordinator will be responsible for providing direct support and coordinating care services for patients. The ideal candidate will have experience in direct patient care, or related education and possess strong organizational and communication skills with an empathetic mindset.
