Overview:
The Care Transition Coordinator is responsible for generating hospice referrals through professional relationship with all referral sources, which may include physicians, nursing homes, assisted living facilities, and hospitals. Must also develop relationships within the community through activities and events while maintaining strong organizational skills, clear communication and an effective understanding of the hospice philosophy.
Responsibilities:
  • Establish and promote positive professional relationships with physicians, hospitals, skilled nursing facilities, discharge planners, and case managers within an assigned territory to promote our hospice services.
  • Educate the community, referral sources, patients and families on hospice services through daily, planned activities including in-services, one-on-one presentations and group meetings.
  • Develop, execute and evaluate strategic marketing plans including tactics to meet budgeted admission goals.
  • Within scope of position, provide leadership and personal accountability to ensure compliance to company and regulatory policies, business growth, professional development and integrity.
  • Collaborate with the patient care team to provide assistance in obtaining information from referral sources and handling stressful situation and deadlines by effectively communicating with the healthcare team.
  • Gather and organize account-related information and provide input on key customer opportunities, and proposed contracting.
  • Take initiative to develop and maintain relationships with team members to assure effective communication both internally and externally for our referral sources.
  • Ensure compliance to regulatory requirements and guidelines, company identity and admissions standards.
  • Performs other activities as assigned.
  • Consistently promotes company values.
  • Utilizes marketing/sales collateral to maintain and develop business opportunities.
  • Assist the program as needed with informational visits with patients and families.
  • Plan and coordinate public education and communication efforts to increase community outreach
  • Ensuring coordination with other departments, services and senior management, as appropriate.
  • Staying within the budgeted expenses set by management for all in-services and activities.
Working Conditions:
  • Ability to move, stand and bend 40-89% of the time if necessary.
  • Will be operating computers and a variety of office equipment.
  • Able to position, move and lift medical supplies up to 50lbs.
  • Must be able to communicate with referral sources and all outside entities telephonically.
  • Ability to operate a variety of electronic devices and participate in webinars and conference calls.
  • Support and direct sales staff and branch leadership with urgent patient related matters that can create stressful situations.
  • Need to be able to focus for long periods of time.
  • Ability to cope with death and dying and the psychological effects.
  • Environmental Requirements:
  • Ability to work in an office setting 0%-20% of the time.
  • Occasionally meet with patients and families in their place of residence or assist with deliveries as needed.
Travel: Ability to travel day and/or some overnight travel, approximately 90-100% of the time or as needed within the Division Service area.
Qualifications:
Education: Bachelor’s Degree in marketing or Business preferred and/or experience in a health-related field or equivalent.
Knowledge/Experience:
  • Minimum of two years health care or related industry sales experience.
  • Excellent communication skills and the ability to effectively communicate hospice services to the community and healthcare professionals
  • Be coachable with a goal to grow personally and professionally.
  • Embrace the hospice philosophy and the principles of death or dying.
  • Has a working knowledge of hospice care and the different aspects that are involved in supporting both the field staff and leadership of the program.
  • Ability to work independently and prioritize.
  • Has an ability to deal tactfully with the community. Understands hospice care and the services provided to patient and family/caregiver through an interdisciplinary group. Intimate knowledge of Medicare Hospice Certification and for the incumbents own professional requirements.

Transportation
: Reliable transportation and current driver's license, required. Valid and current auto liability insurance in accordance with laws of the state, required.
