As a Community Support Worker, you'll partner with family members and existing providers to create and achieve individualized goals that enhance overall independence at home and in the community.
Essential Duties:
- Support individuals in learning, maintaining, and improving skills through participation in various everyday activities, both in homes and the community.
- Implement support strategies to achieve personalized goals for individuals.
- Submit accurate and complete daily progress notes for each shift worked with individuals.
Qualifications:
- High School Diploma or General Equivalency Diploma (GED)
- Valid driver’s license and existing insurance
- Willingness to drive individuals in personal cars throughout the community (mileage reimbursement provided)
- Willingness to comply with required policies, standards, and regulations
- Flexible availability
- Excellent organizational, interpersonal, and communication skills
