KALEIDOSCOPE COUNSELING AND CASE MANAGEMENT, LLC

JOB TITLE: Support Specialist QMHP-A (must- have)

IMMEDIATE SUPERVISOR: MHSS Supervisor

CLASSIFICATION: Independent Contractor

DUTIES AND RESPONSIBILITIES:

  • Manages a small caseload of individual families, providing services on an intensive basis and utilizing short-term, solution focused treatment strategies
  • Interview/document the individual from multiple perspectives, including family functioning, psychological/emotion, social, academic, medical, behavioral, substance abuse, skills, strengths, barriers to economic self-sufficiency, treatment history, and others, and utilizing various assessment instruments
  • Prepares written service plans, incorporating input from individuals, involved service providers, and wraparound team members, and clearly identifying problem areas and needs, strategies and service objectives
  • Facilitates wraparound services to enlist existing supports in determining and meeting service objectives and to identify additional resources and supports.
  • Provides direct clinical services to individuals, including individual (parenting skills, anger management)
  • Collaborates and coordinates with other case-involved specific providers and professionals such as doctors, attorneys, social workers, and probation/parole officers.
  • Responds to crisis situations with twenty-four hour a day availability.
  • Documents case activities for both medical necessity and billing purposes (Quarterlies, ISP’s, SRA’s, Transfers, Clinical Emergencies, Serious Incident Reports, Discharges, etc.)
  • Prepares progress reports, monthly reports as required by referral sources or service purchasers.
  • Responsible for ensuring that individuals' MCO’s remain active during ALL service delivery.
  • Participates in Child Specific Team, Family Assessment and Planning Team, and other interdisciplinary, diagnostic, or planning meetings
  • Links individuals with external programs or services, such as health services, recreational activities, child care services, financial resources, employment resources, AA/NA groups, transportation resources, and others.
  • Attends court hearings when families are court-involved and presents testimony and treatment recommendations as requested by attorneys, referral sources, the court or client guides.
  • Works with the MHSS Supervisor / LMHP to increase the effectiveness of therapeutic interventions
  • Prepares written discharge summary and plans
  • Accepts and responds to both routine and emergency referrals
  • Obtains demographic and clinical information from referral sources
  • Performs other tasks as assigned by the MHSS Supervisor, Chief Executive Officer/ Designee.
  • Conducts the maximum units allowable/necessary for optimal client benefit.
  • Must have a valid driver’s license, computer, cell phone, and registered/operable vehicle.

MINIMUM QUALIFICATIONS:

Education and Experience

QMHP must have at least a Bachelor’s Degree preferred in Social Work, Psychology or Human Services field, (1) one year experience working with adults, children, and families with a mental health diagnosis.

Knowledge, skills, and ability

Skills in program development and design; diagnostic assessment, social work principles and ethics to include confidentiality, relevant therapeutic intervention modalities, motivational interviewing, CBT, reality problem solving, brief solution focus and working knowledge of human services system; relevant community resources; ability to maintain an effective working relationship with other social service providers, service purchasers, and clients; ability to communicate effectively both orally and in writing. Must maintain appropriate training and qualifications to satisfy QMHP- A license requirements with the VABoard of Counseling. Must be CPR/ First Aid certified.
