Salary Range: $66,560 - $70,560 per year, DOE

Office/Service Area Location: Pasadena, CA (service Los Angeles County)

The essential functions include:

  • Coordination of services and case management, discharge planning, case consultation with referral sources, schools, allied agencies, mental health providers, medical providers, education instructors, babysitters and others.
  • Planning for treatment, reunification or permanent placement;
  • Provide Case Management services to children placed in foster homes certified by the Foster Family Agency, developing written treatment plans for the child during the initial 30 days of placement, quarterly thereafter, and a final termination summary;
  • Monitor and ensure appropriate standards of care provided by foster parents to children placed by the Foster Care and Adoptions program.
  • Oversee and/or assist foster parents access, use and compliance to adequate medical care and support services for children placed in their home.
  • Consult with foster parents and children with regard to behavioral and psychosocial adaptation.
  • Adhere to all code of ethics, mandated reporting, and ensure confidentiality of children's information including significant incident reports, CCL and ORR.
  • In coordination with clinician, support a therapeutic caseload of children within the Foster Care and Adoptions program. This includes oversight and management of behavioral health care to include: assessment, treatment planning, support mental health services, administration and analysis of outcome measures to guide treatment.
  • Serve as a liaison with Federal Field Specialists, Care Coordinators, Legal Counsel, and other representatives or agencies in the ORR network.
  • Screens for human trafficking concerns as well as other risk and safety issues that may require crisis intervention, safety planning and/or mandated reporting.
  • Ensure accurate and timely documentation and reporting of suspected child abuse and incident reports as per agency policy and state mandates;
  • Ensure FFA Program compliance with the state, county, federal, licensing standards and regulations; and agency policies;
  • Monitor and ensure appropriate standards of care provided by foster parents to children placed in care.
  • Follow up with resource parents for quality assurance, health, safety and provide psychoeducation.
  • Oversee and/or assist foster parents access, use and compliance to adequate medical care and support services for children placed in their home;
  • Consult with foster parents and children with regard to behavioral and psychosocial adaptation.
  • Conducts interviews/assessments of family members, friends of family and/or sponsors to determine integrity of the relationship and verify information received from minor.
  • Transport and/or support transportation of minors to facilitate program services, including but not limited to, court appointments, attorney visits, airports, other appointments as per contractual requirements as needed. 
  • Supervises and trains new case managers.
  • On Call after hours.
  • Other duties as required.

EDUCATION, EXPERIENCE & CERTIFICATES 

  • A master's degree in the behavioral sciences, human services or social services field. 
  • OR a bachelor's degree in social services related field with at least 3 years' experience of progressive employment that demonstrates supervisory and case management experience.
  • Bilingual in Spanish Required.
  • Valid California driver's license, driving record and personal vehicle insurance acceptable to Hillsides' insurance carrier.
