Job Title: Rural Health Clinic Practitioner/ Hospitalist

Department: Rural Health Clinic

Reports To: Chief Medical Officer

Status: Non-Bargaining, Exempt

Job Summary: To safeguard the health and well-being of patients and KMC staff by providing inpatient hospitalist services and outpatient primary healthcare services. Examine and treat patients with a wide range of conditions and refer those with serious ailments to a specialist or appropriate facility. Job duties include, but are not limited to admitting patients, providing continuous care to hospital inpatients, taking patient histories, performing or requesting diagnostic tests, making diagnoses, prescribing treatment, performing minor procedures and follow-up examinations, as well as educating patients/families/caregivers in disease prevention and health maintenance. Commit to train future healthcare providers to deliver quality health services to patients of diverse cultural, ethnic, and economic backgrounds.

Duties and Responsibilities:

A. Hospitalist:

1. Participate in hospitalist rotation as determined by department.

2. Admitting patients and creating and recommending treatment plans, ongoing assessments, and changes in level of care.

3. Taking patient medical histories, updating patient information and charts with the latest findings and treatments.

4. Ordering tests for health care staff to administer and reviewing and interpreting test results.

5. Participates in quality reporting and improvement initiatives to support patient safety, and infection control standards.

6. May manage the daily operations of a specific medical program, patient care unit, or research function.

7. Responding to questions and concerns from patients/families/caregivers about their health care, well-being, and discharge plans.

8. Discussing proper nutrition, hygiene, and self-care with patients to promote good health.

9. Manage patients on the unit to desired outcomes, including medical crisis, rapid response, level of care changes, and end of life care.

10. Rounding and working with the interdisciplinary team, such as rehabilitation therapists, nurses, social services, behavioral health providers, pharmacists, and dieticians, to develop patient care plans.

11. Interface with emergency department clinicians as needed.

B. Outpatient Services:

1. Provides and manages direct patient care, including physical examinations, evaluations, assessments, diagnoses, and treatment for a specified patient population.

2. Prescribes pharmaceuticals, other medications, and treatment regimens as appropriate to assessed medical conditions.

3. Refers patients to specialists and to relevant patient care components as appropriate.

4. Performs chronic care management and engages in value-based care initiatives to ensure quality care for the local community.

5. Trains and supervises medical students and residents engaged in educational activities and procedures, as appropriate.

6. Follows established departmental policies and procedures, continuous quality improvement objectives, and safety, environmental, and infection control standards.

7. As appropriate to the position, participates in specified health promotion, education and/or prevention programs.

8. Engages in initiatives to identify social risk factors and refers to programs as appropriate.

9. Performs miscellaneous job-related duties as assigned.

C. Additional Job Duties:

1. Complies with all hospital and departmental policies and procedures, safety program requirements, corporate compliance program standards of conduct, all applicable statutes, rules, and regulatory requirements.

2. Enhances professional growth and development and maintains skills and knowledge through participation in educational programs, current literature, in services, workshops and professional affiliations.

3. Performs other duties as assigned or requested.

Required Knowledge/Skills/Abilities:

· Knowledge of legal and ethical standards for the delivery of medical care.

· Strong medical diagnostic, assessment and treatment skills in patient care services for area of medical expertise.

· Ability to supervise, advise, and train clinical professionals and/or students in area of expertise.

· Knowledge of relevant drugs and non-pharmaceutical patient care aids and ability to prescribe dosages and instruct patients in correct usage.

· Ability to develop and present educational programs and/or workshops.

· Knowledge of current principles, methods and procedures for the delivery of medical evaluation, diagnosis and treatment in area of expertise.

· Ability to work both independently and in a team environment.

· Knowledge of related accreditation and certification requirements.

· Ability to observe, assess, and record symptoms, reactions, and progress.

· Effective verbal and writing skills.

· Excellent communication and interpersonal skills.

Education and Experience:

REQUIRED:

· MD, DO, or professional degree in medicine from an accredited medical school, or Physician Assistant (PA) or Advanced Practice Nurse Practitioner (APRN), master’s in nursing with board certification.

· Current unencumbered State of Hawaii MD, DO, PA, or NP license for the state.

· Current BLS, ACLS certification.

· Current State and Federal Narcotics Licenses.

PREFERRED:

· 5 years of experience with hospital medicine

Physical Requirements:

· Prolonged sitting, standing, and walking.

· Continuous eye-hand-foot coordination, with occasional handling and fingering.

· Continuous use of vision (corrected vision, depth perception, wide field of vision and color vision).

· Pushing and pulling to 300 lbs. with assisted lifting and carrying to 150 lbs.

· High attention to detail, concentration, alertness levels with frequent use of mathematical ability.

· Ability to communicate (written, verbal, etc.) clearly and effectively in English.

· May be exposed to blood and bodily fluids.
