- Provide direct patient care, including assisting with activities of daily living (ADLs) such as bathing, dressing, grooming, and toileting
- Administer medications as prescribed by healthcare professionals
- Monitor and record vital signs, such as blood pressure, pulse, and temperature
- Assist with mobility and transfers, using proper body mechanics and assistive devices
- Provide emotional support and companionship to patients
- Follow care plans and communicate any changes in patient condition to the appropriate healthcare professionals
- Assist with meal planning and preparation, ensuring dietary restrictions are followed
- Perform light housekeeping tasks to maintain a clean and safe environment for patients

- Strong knowledge of patient care techniques and medical terminology
- Familiarity with medical systems and equipment used in home healthcare settings
- Experience in providing specialized care for patients with dementia or developmental disabilities
- Proficient in medication administration and able to accurately document medication administration records
- Ability to work effectively with a multidisciplinary team of healthcare professionals, including nurses and therapists
- Knowledge of tube feeding procedures and able to provide appropriate care for patients with feeding tubes
