JWCH Institute is Hiring a Lead Sr. Community Outreach Worker Near Los Angeles, CA

Position Purpose:

The Lead Sr. Community Health Worker is responsible for leading the recruitment efforts and HIV/STD testing of clients interested in the CARE project. More specifically, this position will lead targeted outreach to at-risk MSM and transwomen, ages 18 and over in Los Angeles County (LAC). 

Principal Responsibilities: 

  1. Responsible for outreach, recruitment and engagement targeting adult MSM and transwomen. 
  2. Conduct a community needs assessment and processes for program improvement of services focusing on MSM of color, provided by CARE projects.
  3. Conduct HIV/ STD testing.
  4. Provide leadership at agency and community-levels in regards to HIV related topics.
  5. Provide linkage to care services to newly diagnosed HIV-positive MSM and re-engagement to persons who are out-of-care. 
  6. Attend monthly networking meetings as representative of JWCH Institute, Inc. Wesley Health Centers.
  7. Provide Biomedical HIV prevention services including PrEP/PEP.
  8. Assist with the preparation of monthly and annual reports according to CDC deadlines and guidelines.
  9. Assist with the development social marketing material, including social media, for MSM of color.
  10. Perform consistent program evaluations, implementation of data collection tools and the management of data including, but not limited to, adherence to HIPAA, patient/client confidentiality and data security measures. 
  11. Provide client follow-up, monitoring of data quality, and data entry as necessary.
  12. Conducts group and advisory committee meetings.
  13. Trains and mentors new Community Outreach Workers and Peer Health Educators. 
  14. Responsible for coordinating, planning and implementing program outreach events.
  15. Prepares program calendar, schedule and other time-sensitive documents in a timely manner. 
  16. Other duties as assigned.

Requirements:

  1. 2-3 years’ equivalent experience conducting outreach, health education, testing and recruitment of MSM.
  2. 2-3 years’ experience in HIV prevention targeting MSM of color.
  3. Current HIV/AIDS and STD knowledge.
  4. Effective oral and written communication skills.
  5. Proficient and knowledgeable of Microsoft suites such as word processing and excel.
  6. Possess a valid California Driver’s License and ability to drive to and from program sites.

Preferred: 

  1. Experience in working with HIV evidence based interventions.
  2. Knowledge of social factors impacting MSM of color.
  3. Obtain a current HIV Basic II Certification.
