JWCH Institute is Hiring a Community Health Worker - H.A.R.R.P Near Los Angeles, CA

Position Purpose:

Under the direction of the program manager, the Community Health Worker will assist in implementing the Healthy Alternatives to Reducing the Risk for HIV Infection Program (H.A.R.R.P) by recruiting clients, conducting risk assessments and providing educational sessions to homeless individuals in the Downtown Skid Row area. The Community Health Worker will be engaging and developing relationships with at risk and/or homeless individuals with a variety of needs, including but limited to chronic health conditions, substance use, behavioral and mental health issues, and other social service referrals and linkages. 

Principal Responsibilities:

  1. Actively seek opportunities for outreach with a focus in identifying hard to reach populations. 
  2.  Perform outreach activities to assist clients to participate in the H.A.R.R.P. behavioral change program. 
  3. Develop a rapport and establish trusting relationships with clients during the H.A.R.R.P HIV/AIDS behavior change sessions that reinforces a positive lifestyle.
  4. Participant in clerical duties such intake assessments and coordinating appointments for clients. 
  5. Engage with the homeless population to promote participation in the ongoing reinforcement of various prevention skills.
  6. Provide health education and help implement the H.A.R.R.P curriculum at various outreach sites.
  7. Assists in gathering and compiling program data for the Health Education Coordinator.
  8. Provide support and help participants to build emotional and social support systems that motivates them to change their high-risk behaviors.
  9. Participate in JWCH trainings and advisory committee meetings as scheduled. 
  10. Performs other duties, as needed.

Requirements:

  1. Strong ability to develop positive relationship with clients living in temporary housing (e.g. shelter; mission; hotel or substance abuse recovery facility).
  2. At least one year experience in outreach activities, either through lived experience, volunteering or employment. 
  3. Excellent verbal communication skills.
  4. Ability to work with homeless; mentally ill and/or substance abusing adults as well as professional staff.
  5. Access to transportation to and from office and all program related sites.
  6. Reliability in performance of job duties; timely arrival at all sessions.

Preferred:

  1. Experience working with homeless population.
  2. Successfully completed and received certification from the H.A.R.R.P Behavior Change Program.
