PART-TIME IN-HOME CASE MANAGER

Family Resource Specialist

$500 STAY BONUS

Duties & Responsibilities:

Assess and engage families in service and treatment planning based on demonstrated needs.
Plan and implement appropriate interventions using evidence based and best practice approaches.
Assist families in accessing and navigating community resources.
Meet with designated clients for the prescribed number of hours/ days/contacts per week.
Maintain open communication with referring authorities.
Attend court hearings and provide testimony as required.
Submit accurate, required notes within 24 hours of service and complete reports within assigned timeframes.
Transport clients utilizing personal vehicle, as necessary.

Requirements:

Bachelor's degree and experience working with children, youth, and/or families in a variety of settings or environments.
Valid driver’s license, auto insurance and access to a vehicle.
Pass current, applicable clearances.
Pass pre-employment, post-offer drug screening.
Ability to work non-traditional hours including evenings and weekends.
