We are seeking a skilled and compassionate Nurse to join our healthcare team. Being a UAS Nurse requires strong assessment and communication skills, as well as a deep understanding of various health conditions, disabilities, and community support services.
UAS Nurse Responsibilities:
Perform UAS Reassessments in client's home or facility according to state guidelines.
Document any concerns, conflicting information, other issues that surface during the evaluation process.
Enter data into electronic UAS evaluation form.
Assess eligibility in need of Long Term Care Services
Ensuring compliance with regulatory requirements and healthcare industry standards
Assisting in the coordination of interdisciplinary teams to ensure continuity and quality of patient care
Providing education and guidance to patients and their families regarding healthcare resources and options available to them
Reviewing patient medical records and assessing their clinical condition to determine the appropriate level of care
UAS Nurse Qualifications:
UAS Certified (Uniform Assessment System for New York)
Minimum 1 year preferred UAS / Home Care Experience
RN License / New York State Nursing License
Ready to work and dedicated
In person cases
Pay: $120-200 per reassessment visit (Fee for Service)
Manhattan, Bronx/Westchester, Staten Island, Queens & Brooklyn Available!
