LPN Wellness Nurse:

$38-42/hr

Juniper Canyon Living is a 108-unit special contract residential care facility campus designed and staffed uniquely for residents who have failed placement elsewhere due to complex behaviors and/or medical needs. We are looking for a Full Time LPN Wellness Nurse to work alongside our Director of Nursing to provide exceptional resident care. This position is Day shift, Monday-Friday, 7am-3:30pm.

The primary responsibilities of the LPN Wellness Nurse include, but are not limited to:

• Working hand in hand with our Director of Admissions to conduct pre-Admission assessments/screenings.
• Provide oversight of resident's Service Plan most especially Quarterly.
• Evaluations and Significant Changes of Condition.
• Monitor and assess resident physical and cognitive functioning.
• Frequent communication with families and external care support teams.
• Coordination with the pharmacy, physician, resident and/or family to ensure accurate medication orders.
• Train, supervise, lead, and motivate the team to bring the highest quality of care to residents.
• Administration and documentation of medications and other treatments.
• Administer non-delegated tasks such as Intramuscular Injection.
• Other duties as Assigned.
