Juneau County, WI is Hiring a CSP Case Manager Near Mauston, WI

Description

  • Write In-depth Assessment, diagnose and develop and individualized Treatment Plan
  • Provide supportive psychotherapy, insight oriented therapy and symptom assessment for community support clients.
  • Facilitate the treatment of and maintain the SPMI population within the community, acting as Case Manager.
  • Provide 24 hour crisis intervention for adults and children and evaluate the need for psychiatric hospitalization.
  • Provide psycho education, support and consultation to families and adult family home or CBRF staff.
  • Act as liaison to hospital staff to coordinate treatment of community support clients.
  • Facilitate Chapter 51 and 55 commitments and protective placement. Acting as liaison with legal system
  • Oversee activities of paraprofessional staff (Mental Health Technician) providing direction and guidance.
  • Act as liaison to facilitate referrals to, and coordinate client services, both internally and with outside agencies.
  • Assume responsibility for a high level of team work including participating in weekly meetings and staffing.
  • Responsibility of developing, implementing and overseeing Treatment Plans for clients.
  • Responsible for documenting client related contacts including Treatment Plans, bi-monthly Progress Notes, etc.
  • Attend in-services and trainings related to CSP Services.
  • Assume responsibilities in coordinating appropriate living arrangements.
  • Other duties as assigned.

Examples of Duties

  • Bachelor Degree in Social Work, Psychology or Human Services related field, or ability to obtain such within six months of hire. Other bachelor's degrees may be considered. 1,000 hours of supervised, post degree experience in working with chronic mentally ill preferred. Social work licensure or eligibility for licensure preferred.
  • Appropriate licensure or certification preferred. Individual lacking this credential will be expected to make an ongoing diligent effort to attain the credential.
  • Knowledge of social work theory and practices.
  • Knowledge of social, psychological and biological problems affecting individual’s mental health.
  • Knowledge of DSM IV and use of assessing and diagnosis.
  • Knowledge of community treatment theories and practices for people with long term mental illness.
  • Ability to do In-depth Assessment, develop diagnostic impression and write individualized treatment plan.
  • Ability to establish good rapport with clients.
  • Ability to work with others internally and externally.
  • Ability to communicate effective orally and in writing.
  • Ability to give direction and guidance to paraprofessional staff.
  • Ability to provide training in ADL skills.
  • Must possess valid driver’s license
