Our Hospice team has an immediate opening for a an Intake Coordinator to support our intake and admissions teams. Job Responsibilities include but are not limited to:Taking referrals via phone, fax or email and entering them into the EMR, auditing admissions for compliance, obtaining certifications and past medical history from physiciansEvaluating appropriateness of all referrals for information visit/admission assessments; referring to other community resources and JSSA as appropriateTriaging needs of hospice admission nurses while in the field Obtaining Verbal Certifications from Physicians for start of care of hospiceActing as an ambassador for the Hospice Program and our organization when communicating with patients, families, and physicians.Able to work a flexible schedule of four 10 hours days including every other weekend.Position Qualifications:Minimum of 2 years of intake experience (within hospice, homecare of similar healthcare environments preferred)Strong written and oral communications skillExcellent computer skills – Microsoft Office 365, EMRs, databases, and moreExcellent phone customer service skillsStrong organizational and multitasking skillsAbility to work in a fast-paced environment with an emphasis on collaboration and maintaining positive relationships with colleagues and clientsAbility to actively listen and concisely explain complex information to clientsBenefits:Supportive work environmentComprehensive benefits package to include: Medical/Prescription/Dental/Vision insurance, 403b Retirement Plan, Paid Leave (sick/vacation/personal/holidays), and much more...All staff must comply with our mandatory COVID vaccination policy requiring staff to be up to date on their vaccinations.#GC
