Job Summary: The Behavioral Health Care Management Coordinator is responsible and accountable for the coordination of patient care across the care continuum with an emphasis on patient stabilization in the least restrictive setting and the outcome of reducing acute behavioral health readmissions.

Essential Job Functions & Accountabilities:

  1. Coordinates the care of an identified behavioral health population throughout the healthcare continuum.
  2. Meets with the team and contributes to the development of a comprehensive plan of care based on a systematic assessment of the physiological and psychosocial needs of the patient.
  3. Evaluates and modifies plans of care as needed. May provide ongoing follow-up between hospitalizations/outpatient visits.
  4. Monitors patient progress towards established goals. Regularly communicates changes to the patient, family, healthcare team and other agencies involved.
  5. Collaborates with patient, family and healthcare team to arrange for identified home care needs. Acts as a hospital liaison for agencies and individuals involved with selected patients.
  6. Consults on patient care issues in area(s) of expertise. Functions as a resource to patients and families. May perform specific assessments or procedures for patients if trained and competent to do so.
  7. Documents all patient care activities appropriately in the medical record.
  8. Develops and maintains data system to track patients and outcomes.
  9. Identifies quality Improvement initiatives.
  10. Maintains timely, effective communications with the department leadership regarding all events or proposed changes which might impact regulatory issues, census variance, policies or procedures.
