Job Description:

Job Summary: The TCHATT Clinical Referral Specialist is responsible for the clinical evaluation of all referrals received by the department. The Clinical Referral Specialist coordinates referrals with school employees, TCHATT providers, and leaders to ensure clinical fit for the TCHATT program, and then processes referral for smooth and efficient coordination of care. This includes assigning appropriate TCHATT case managers and therapists to the referral.

Essential Job Functions & Accountabilities:

  • Monitors electronic systems for patient referrals from schools.
  • Maintains accurate tracking of referral status and processes.
  • Gathers pertinent patient information from a variety of resources (e.g. schools, patients, family members etc.).
  • Makes clinical decisions regarding fit of referral for the TCHATT program.
  • Coordinates referrals and consults with other service providers, as well as other community resources, as appropriate.
  • Coordinates referral approval with provider.
  • Coordinates with schools to gain additional clinical insight into referral or, if not a program fit, to discuss reason for declining referral, and provide appropriate resources as necessary.
  • Coordinates patient care with other team members and providers for scheduling of approved patients, including assigning referrals to therapist, and case manager, as needed.
  • Charts assigned patients per operating procedure and maintains other records as assigned.
  • Maintains current knowledge of community resources/information and issues affecting the populations served.
  • Maintains competency relative to Social Work services for population served. Exhibits professional growth and development.
  • Assumes, at times, responsibility for psycho-educational services or other responsibilities as assigned.

Qualifications:


Required Qualifications:

  • Master’s Degree in Social Work, Counseling or related field of study.
  • Must have a current Licensed Master Social Worker (LMSW), Licensed Clincial Social Worker (LCSW) licensure, or Licensed Professional Counselor (LPC) from the Texas State Board of Social Work Examiners.
  • Current JPS recognized Cardiopulmonary Resuscitation (CPR) certification and must maintain throughout employment. May be obtained subsequent to hiring.
Preferred Qualifications:
  • 2 plus years of professional social work experience in an outpatient behavioral health care setting performing any combination of: coordinating patient acceptance, continuity of care planning; conducting initial screenings and evaluations of patients and families; providing or making referrals for individual, family, and group psychotherapy; and/or promoting patient navigation services.
  • Licensed Clinical Social Worker (LCSW) licensure or Licensed Professional Counselor (LPC) from the Texas State Board of Social Work Examiners.
