Job Description:

Job Summary: The RN Patient Placement Coordinator (PPC) assesses patient admissions, discharges, and inpatient bed availability. This job also develops, coordinates, and implements plans for patient assignments that accommodate patient needs, unit workload, and physician preferences.

Essential Job Functions & Accountabilities:

Assumes roles and responsibilities for all three roles; Front flow, Backflow or Behavioral Health Flow Coordinator, as applicable.
Facilitates and manages patient flow throughout the department to ensure that patients are expeditiously moved into the treatment area and wait time is minimized.
Coordinates with Physicians to identify patients eligible and appropriate for Results Pending to facilitate movement of patients before, during, and after determination of treatment needs. Assist with discharges as appropriate.
Facilitates movement of patients to other areas/units and procedures.
Evaluates the admission and transfer process for patients, families, and physicians on an individual and collective basis.
Reviews all admissions including the charting of the primary nurse for completeness, fax report to the receiving units, and makes preparation for the patient’s immediate transport to the receiving unit. If working in Women’s department, serves as primary contact for all admissions.
Maintains knowledge of the actual and potential disposition status of the patients within the applicable unit.
Provides support and resources to staff and physicians.
Respond to all Priority Alerts in the department.
Identifies barriers to the admission and discharge process and suggests solutions in conjunction with the Hospital Leadership Team.
Contributes to the achievement of the department’s metrics including but not limited to: door-to-discharge, bed assigned-to-discharge, and Left without Being Seen rate.
Attends all Bed Control meetings while on duty to represent the interest of the patient.
Participates as a member of the Leadership Team, is Team Leader capable and attends leadership meetings/committees.
Participates in all huddles to facilitate throughput plan and movement of patients as needed.
Functions as needed as the backup for triage, to support the flow of new patient arrivals to the unit.
Assists with the care of patients by way of assessment, treatment planning, and medication administration.
Works in collaboration with unit TLs to ensure adequate and appropriate staffing is available for safe patient care. Staffing concerns are escalated to appropriate leaders according to department expectations. Works with support departments to ensure quality services and adequate coverage.
Rounds on all areas of the unit throughout shift.

Qualifications:


Required Qualifications:

  • Associate’s Degree from an accredited school of professional nursing.
  • 2 plus years of nursing experience.
  • Current licensure by the Board of Nurse Examiners for the State of Texas or proof of reciprocity of licensure between the State of Texas and another state.
  • Current JPS recognized BLS certification and must be maintained throughout employment.
  • Successful completion of background screening.
  • If working in the Women’s department, must also have the following:
  • Current JPS recognized Cardiopulmonary Resuscitation (CPR) certification and must be maintained throughout employment.
  • Advanced Cardiovascular Life Support (ACLS) certification with 6 months of employment.
  • Current Neonatal Resuscitation Program (NRP) and maintains credentialing throughout employment.
Preferred Qualifications:
  • Bachelor of Science Nursing Degree from an accredited school of professional nursing.
  • Prior working experience in the applicable department.
  • National Nursing Certification in related specialty area.
  • If working in the Women’s department, AWHONN Fetal monitoring certification (required if clinical experience includes fetal monitoring.) and Basic EKG.
