Jovenes, Inc. is Hiring a Connect LA - Case Manager Near Whittier, CA

Jovenes is a place of personal transformation for homeless youth. Our mission is to help homeless youth and at-risk families become productive and integrated members of the community. We provide emergency and permanent housing options and other community-based services that create opportunities for them to lead productive lives. We operate in Los Angeles and the community East and Southeast of Los Angeles County.

JOB TITLE: Connect LA Case Manager

The goal of Youth Connect LA Program (formerly known as Family Reconnection”) is to assist Transition Age Youth (TAY) with reconnecting with family and/or building/strengthening positive & healthy relationships with adults. The position will work closely with the Program Manager and Outreach Team to help client and family members overcome obstacles and challenges that may prevent client from having a relationship. The service plan will focus on building healthy relationships, strengthening adult connections, reunification with family, Case Management & Supportive Services, as well as employing a Trauma Informed Care, Housing First and Harm Reduction approach. This program is part of Jovenes’ Regional Coordination for Service Planning Area and serve communities East and South- East of Los Angeles County. DUTIES AND RESPONSIBILITIES

Supportive Services:

· Conduct screening interviews, complete intake documentation and coordinate move in and exit of all program participants.

· Provide case management and family re-unification services designed to assist clients obtain housing stability.

· Perform initial comprehensive case management assessment and develop a plan of intervention that addresses need for housing, services, crisis intervention, and continuity of care planning and coordination.

· Manage financial support for the client.

· Provide onsite case management services in the areas of independent living skills, employment linkage, benefits establishment, and other services as needed to assist in reaching case plan and treatment plan goals.

· Respond to referrals and client’s request for case management assessment and intervention within required response time.

· Encourage and promote an environment that is strength based to assist clients in meeting their individual goals.

· Develop an aftercare support plan.

Contract Management:

· Achieve knowledge of the contracts and grants working under.

· Maintain client related data tracking systems, including case notes and complete HMIS entries.

· Prepare case-related reports including outcomes, successes and challenges.

· Generate client data for monthly reporting.

· Maintain complete and accurate documentation of service objectives and outcomes as well as other services in accordance with federal, state, county and PATH guidelines

· Complete follow-up and retention services, and provide back-up documentation in client file.

· Outreach and Relationship Management

· Outreach to community, business owners, realtors, landlords, housing developers and other service providers to identify new and existing opportunities and build strong relationships to better assist clients in accessing resources, employment, supportive services, and housing opportunities.

· Respond to community requests for street outreach intervention.

· Mediate disputes between homeless persons and neighborhood residents.

· Attend collaborative meetings.

· Network with other agencies, coalitions, and local community meetings.

· Actively participate in staff meetings and trainings.

· Other duties as assigned

In the course of performing the job, this position typically spends time sitting, standing, climbing stairs, walking, driving, carrying (15lbs), and lifting (15lbs). Coordinate multiple tasks simultaneously.

Perform some repetitive motion activities. Operates a computer keyboard, adding machine; firm/strong grasping required. Makes and receives telephone calls. Daily use of general office equipment copier, fax machine, phones, etc.

Work Environment:

You may be in contact with individuals and families in crisis who may be ill, using substances and/or not attentive to personal health and safety for themselves. The employee may experience a number of unpleasant sensory demands associated with the client’s use of alcohol and drugs, and the lack of personal care. The employee must be ready to respond quickly and effectively to many types of situations, including crisis situations and potentially hostile situations.

Education, Experience, Certificates & Training:

  • Bachelor’s degree in social work, counseling or similar or 4 years of experience in a relevant mental health setting;
  • Experience working with transitional age youth, individual with mental disabilities, low income families.
  • Demonstrated ability to work with a culturally diverse clientele & staff
  • Valid California driver’s license and personal vehicle insurance.
  • Fluent in Spanish

Level of experience and education will be taken into consideration. Health, dental and life insurance are provided. Vision, accidental, critical illness, and additional life insurance are available on a complimentary basis. Vacation, sick time, and holidays are provided based on agency policies. 401k plan and matching contributions are provided based on agency policies. Must have a reliable motor vehicle, valid driver license and proof of liability insurance for use of personal vehicle for work-related travel. Mileage reimbursement is provided.
