JOL Healthcare is Hiring a Hospice RN Near Bastrop, TX

The Registered Nurse is responsible for administering quality, skilled nursing care to patients from diverse populations in various home environments. The RN assumes responsibility for coordinating care with the patient and family, certifying the provider, and all interdisciplinary team members. The RN performs comprehensive assessments of patients and collaborates with the certifying provider to develop a Plan of Care that meets the needs of the patient.

Key Responsibilities

  • Performs initial and ongoing patient assessments and coordinates with the certifying provider to develop a Plan of Care that addresses the patient’s physical, psychosocial, and environmental needs. Completes Outcome and Assessment Information Set (OASIS) assessments at appropriate times.
  • Coordinates services. Informs the physician and all members of the patient’s interdisciplinary team of changes in condition and needs. Communicates with all physicians involved in the plan of care and integrates orders from all relevant practitioners into the plan of care.
  • Initiates appropriate nursing interventions, provides patient and caregiver education prepares patients and caregivers for discharge from services. Implements infection control measures to protect both the staff and the patient.
  • Monitors assigned cases to ensure compliance with third-party payor requirements.
  • Prepares clinical progress notes and completes documentation promptly to ensure continuity of patient care.
  • Provides effective communication to patients, family members, Agency team members, and other healthcare professionals.
  • Supervises teaches, and provides clinical direction to other nursing personnel. Supervises LVNs and home health aides providing services to patients. Conducts aide competency evaluations, if qualified by 2 years RN experience and 1 year home health experience.
  • Demonstrates commitment and professional growth by participating in Agency-sponsored in-services and maintaining or improving competency. Participates in the Agency’s QAPI program.
  • Performs on-call responsibilities and provides on-call services to patients/families as assigned.
  • Promotes the Agency’s philosophy and administrative policies.

Salary:

$90k to $95k annum

Qualifications

  • Associate, or Baccalaureate degree in nursing
  • Current, unencumbered license to practice Registered Nursing in Texas
  • Current CPR certification
  • Reliable transportation

Job Expectations:

Ability to work in patients’ homes in various conditions; possible exposure to blood and bodily fluids and infectious diseases. Must be able to work on a flexible schedule and have the means to travel locally. Some exposure to unpleasant weather.
