SUMMARY: The Palliative Care Social Worker is responsible for conducting psycho/social assessment/evaluations. The Palliative Care Social Worker will collaborate with professional palliative care team to review patient care; provide casework and group work for patients and their families; identify community resources, and assist patients and their families in connecting to these resources. This individual serves as a multidisciplinary patient care team member with liaison responsibilities with the hospital and staff members of community organizations and other healthcare organizations.

EDUCATION AND EXPERIENCE REQUIRED:

1. Licensed to practice as a Social Worker in the District of Columbia. Master’s Degree in Social Work from an accredited school.
2. Minimum of 2-5 years of social work experience within palliative care
3. Current CPR Certification required
