As a Social Services Manager, you will serve as an advocate for residents and patients in a long-term care and short-term rehabilitation / sub-acute care setting, having administrative authority and accountability for the provision of their psycho-social needs. You will coordinate their plan of care, treatment goals, and discharge planning, as well as provide oversight of the center’s Social Services Coordinator(s). *Qualifications:*
* Bachelor’s degree or above in Social Work (BSW) or related field required, such as Human Services, Psychology, Sociology, Gerontology, Special Education, and/or Rehabilitation Counseling.
* One (1 ) years’ supervised social worker experience working directly with individuals in a medical / health care setting required.
* Prior Long-Term Care, Skilled Nursing, or related healthcare social work experience strongly preferred.
