We are in search of a dedicated, compassionate, and experienced Permanent Registered Nurse for Home Care to join our dynamic healthcare team. This position is an excellent opportunity for a Registered Nurse (RN) who is passionate about providing superior medical care to patients within the comfort of their homes. The candidate will be responsible for monitoring patient conditions, administering medications, and collaborating with other healthcare professionals to optimize patient care. This role requires a high level of professionalism, empathy, and the ability to handle complex medical situations effectively.
Responsibilities:
1. Conduct initial patient assessments and develop individualized care plans in accordance with physician orders.
2. Provide a range of nursing services, including wound care, medication administration, pain management, and health education.
3. Monitor patients' health status, document observations, and report any significant changes to the healthcare team.
4. Collaborate with physicians, therapists, and other healthcare professionals to ensure comprehensive patient care.
5. Educate patients and their families about their conditions and teach appropriate disease management techniques.
6. Coordinate with Home Care Intake to facilitate patient admission and discharge processes.
7. Provide Basic Life Support (BLS) in emergency situations.
8. Adhere to all professional, ethical, and legal regulations set by the healthcare industry.
Qualifications:
1. An active Registered Nurse license in good standing.
2. A minimum of 2 years of nursing experience, preferably in home care.
3. Proficient in Patient Care, including wound care, medication administration, and health education.
4. Experience with Home Care Intake, including patient admission and discharge processes.
5. Certification in Basic Life Support (BLS).
6. Exceptional interpersonal, communication, and documentation skills.
7. Ability to work independently and handle a variety of medical situations with professionalism and empathy.
8. Strong organizational skills and the ability to prioritize tasks in a fast-paced environment.
9. Proven ability to maintain patient confidentiality and adhere to ethical standards.
10. Physical stamina to perform nursing duties, including lifting and moving patients when necessary.
