JMHC Brand is Hiring a Community Program Support Specialist Near Madison, WI

Help us build a new Community Support Program team at Journey!!! Community Support Programs offer coordinated, evidence-based care and treatment for adults who live with a severe mental illness. Services are provided in the community and aim to meet an individual’s unique needs, reduce mental health symptoms and challenges in community living, and lead to recovery. Program staff focus on the values of hope, collaboration, personal choice, and respect in their work with those receiving CSP services.

The Program Support Specialist/Mental Health Technician (PSS) assists with the organization, administration, and coordination of the operations of the community support program. The PSS/MHT is primarily an administrative position but includes clinical responsibilities as you interact with program consumers. This position is in a State of Wisconsin certified Community Support Program (DHS 63). The PSS/MHT participates as a member of an interdisciplinary treatment team, who work together in helping each consumer achieve independence and successfully live in the community. The PSS/MHT’s primary duties include: providing in-person, in-office, and telephone support to consumers and visitors, maintaining and organizing staff and consumer’s daily schedules, and updating lists and managing the consumer’s electronic medical record. The PSS/MHT serves under the direction of the Clinical Team Manager. The PSS/MHT will also complete timely and professional documentation of applicable clinical contacts with program consumers. Annual Service Hours expectation: 300 hours adjusted for hours worked.

RESPONSIBILITIES:

A. Administrative (High responsibility: 35% of time)

  1. Organize and maintain team and consumer and daily schedules.
  2. Maintain clinical records.
  3. Maintain spreadsheets and other documents for tracking paperwork and clinical needs, manage team SharePoint sites, team calendars, and compile and update lists.
  4. Complete high quality clinical documentation in a timely manner

B. Programmatic (High responsibility: 50% of time)

  1. Serve as an initial contact for consumers and visitors who come into the office or call the office, and provide triage, support, communication to the team on their behalf, and excellent customer service.
  2. Observe consumers taking medications, distribute money, provide clinical support and other items needed.
  3. Complete accurate clinical documentation of clinical observations and services provided in a timely manner.
  4. Assist staff in establishing priorities for responding to individuals in crisis when there are simultaneous requests both on-site and on the phone.
  5. Answer telephones, provide positive support to consumers utilizing general knowledge of consumers, understanding of their illnesses, comprehension of the mental health system and of community resources, and cultural sensitivity, all while maintaining and adhering to confidentiality guidelines. Document clinical contact with consumers.
  6. Attend and participate in team meetings and trainings.
  7. Coordinate activities and/or meetings with other Program Support Specialists.

C. General Program and Agency Responsibilities (High responsibility: 15% of time)

  1. Design, develop, update and print program forms and brochures.
  2. Coordinate and order supply inventory.
  3. Coordinate building and vehicle maintenance needs.
  4. Work actively and positively to maintain good working relationships between other JMHC programs, vendors and other community members.
  5. Work to improve own cultural sensitivity by participating in the development and implementation of program strategies to continue to improve the cultural sensitivity of the program’s services.
