**Job Summary:**

We are seeking a dedicated Social Worker or Mental Health Counselor to join our team. The ideal candidate will provide professional therapeutic services to patients, focusing on individual, family and group therapy. This role requires expertise in behavioral therapy, patient assessment, and diagnostic evaluation. Reimbursement is Fee-For-Service and based on percentage. Our current location is in Liberty, New York and we are looking to expand to the Middletown area as well.

**Responsibilities:**

- Conduct diagnostic evaluations to assess patients' needs

- Develop and implement individualized treatment plans

- Provide therapy including group therapy and behavioral therapy

- Offer individual and group counseling sessions

- Maintain accurate and up-to-date medical records

- Utilize applied behavior analysis techniques when working with patients

- Collaborate with healthcare professionals to ensure comprehensive care for patients

**Requirements:**

- Bachelor's or Master's degree in Social Work, Mental Health Counseling or related field

- State licensure as a Social Worker, LMHC, LMFT. Provisional licenses will be considered as well.

- Proficiency in patient assessment and developing treatment plans

- Experience in individual and group counseling

- Strong understanding of applied behavior analysis principles

- Excellent communication and interpersonal skills

- Ability to work effectively within a multidisciplinary team
