Registered Nurse Home Health Assesor

Responsibilities:
- Provide comprehensive evaluation services to patients with long-term care needs
- Collaborate with interdisciplinary healthcare team members to develop and implement individualized care plans
- Conduct assessments and evaluations to determine appropriate treatment options
- Coordinate and facilitate communication between patients, families, and healthcare providers
- Monitor patient progress and adjust care plans as needed
- Advocate for patients' rights and ensure access to necessary resources and services
- Document patient interactions, treatment plans, and progress notes accurately and timely

Skills:
- Valid RN license in Georgia
- Minimum 3 years of experience in behavioral health care settings
- Strong assessment and critical thinking skills
- Excellent communication and interpersonal skills
- Ability to work collaboratively in a team environment
- Proficient in electronic medical record systems
- Knowledge of relevant regulations and guidelines related to case management
