Description

POSITION SUMMARY

The role of an Integrated Behavioral Health Clinician at JFS is to be a member of the primary care team assessing behavioral health, substance abuse issues and providing brief interventions for chronic medical conditions. The integrated BH clinician will provide high quality clinical services consistent with the agency mission and professional standard practices. The integrated BH clinician will utilize evidence based therapeutic skills to address the client’s presenting issues and promote healing and growth.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Collaborate with a multi-disciplinary team to provide integrated behavioral health care and advance care management for patients accessing primary care services
  • Assists the primary care provider in recognizing, treating and managing mental health and psychosocial issues affecting chronic medical conditions and acts as a contributing member to the primary care team.
  • Provide brief treatment and crisis intervention to create stability of medical conditions and behavioral health concerns. Develop individualized treatment plans to include additional brief counseling interventions or referral to specialty behavioral health, as needed.
  • Help clients identify and address barriers to treatment compliance and behavioral change to implement healthier lifestyle choices related to their physical and mental health.
  • Demonstrate competence in brief assessment, crisis management, triage, diagnosis, as well as, mental health/substance abuse treatment within a brief solution-focused model.

Qualifications


  • LCSW, LPC, PhD, or LMFT
  • Ability to convey empathy and concern for individuals and their families
  • Ability to be flexible to handle the versatility of the position and its periodic crises
