GERIATRIC CARE MANAGER - JEWISH SERVICES
PALM COAST
Job Summary:
The Care Manager plans and coordinates the care of the elderly and/or disabled to improve their quality of life and to maintain their independence. The Care Manager is an experienced guide and resource for families who may play many roles but is fundamentally loyal to the older or vulnerable person who is the client. The Care Manager develops and maintains good working relationships with family members and other professionals involved in the client’s care. They advocate for clients within complex systems of public and private social services and health care providers.
The incumbent is also responsible for implementing a marketing plan and strategies to increase awareness of care services and create revenue producing opportunities in the greater Jacksonville community. This includes working with the Marketing department to develop materials for clients and the public.
Education:
Bachelor’s degree in Social Work or related field from an accredited college or university
Masters Degree in Social Work preferred
Experience:
Required
Minimum three to five years full time work experience in care management or related fields
Experience working with seniors, families, etc.
Experience with case management, program coordination, and assessment
Excellent verbal and written communications, interpersonal and presentation skills, especially public speaking
Working knowledge of Microsoft Word and Excel
Preferred
Experience working directly with the Jewish community
Knowledge of Jewish practices and customs
Position Description:
Clients and Client Family Members:
Conducting comprehensive in-home assessments to identify needs.
Putting a care plan together and then executing that plan.
Maintaining accurate client files according to Agency standards
Developing care plans and objectives and required documentation .
Acting as a liaison to families at a distance.
Assisting with moving their clients to or from different care settings.
Reviewing financial, legal, or medical issues and referring clients to experts for same such as estate planning, living wills, and family trust experts.
Providing crisis intervention.
Providing client and family education.
Visiting clients on a regular, routine basis to make sure they are safe, doing well, eating properly, and taking needed medications.
Making necessary medical appointments and assuring client gets to them.
Identifying agencies and/or social services and other programs that client can avail.
Coordinating and communicating between all care service providers
Holocaust Survivors
Working with Holocaust survivors to share information about services available to them at JFCS and through the Claims Conference
Preparing survivors' required documentation supporting claims for reparations from Germany
Maintaining relationships with staff at Claims Conference to jointly support Holocaust survivors' receipt of services and/or funds.
Interfacing with home health vendors who provide services to Holocaust survivors to ensure all billing is accurate.
Assisting in planning and conducting social events for Holocaust survivors and their families
Marketing and Development
Instilling a marketing bias throughout the care management service
Developing the marketing strategy and plan
Researching and reporting on external opportunities
Understanding current and potential clients
Identifying revenue goals and setting objectives to achieve outcomes
Coordinating marketing activities to promote services
Developing strategic relationships
Working with local senior facilities, elder care professionals and others in related fields to promote JFCS senior services
Research opportunities for diversifying funding streams
Other duties as assigned.
