Intake Clinician: JFCS is currently seeking an Intake Clinician to complete comprehensive assessments for clients ranging from child/adolescent, adult, and birth to 5-years-old.

Duties include:

• Provide a written clinical summary and provisional individualized service plan as a result of the assessment.
• Utilize the current DSM to provide provisional diagnoses.
• Document all clinical activities the day the service is provided in the electronic medical record.
• Pool Intake Clinician services will be provided across all four Valley clinics.

**Payment ranging from $50-$100 per assessment, based on licensure level, with a flat rate for any onsite training requirements.**

Qualifications:

• Master’s-Level Clinician
• Licensed at associate or independent license preferred.
• Ability to engage members in-person and through telehealth.
• Excellent written/communication skills.
• Ability to work a flexible schedule.
• Ability to work collaboratively with existing clinical leadership to ensure timely access to needed services for members.
