General Responsibilities:

Through an on-site interdisciplinary team of professional staff, JASA will offer comprehensive services to the residents of eligible units to effectively manage health and behavioral health conditions, address other disabling conditions or life challenges and become, and remain stably housed.

Under the supervision of the Associate Director of Residential Services, the Social Worker is responsible for providing direct services to eligible tenants. This includes supportive counseling, medical care coordination, case management, crisis intervention, and assistance with benefits/entitlements.

Specific Duties and Responsibilities:

  • Provides monthly in-home assessments of the social and emotional needs of the tenant.
  • Assists formerly homeless tenants as they transition back to residing in the community.
  • Develops and implements a written case plan to provide a full range of social services for the tenant and their family including:
    • Provides individual and family counseling/guidance to resolve any issues the tenant is facing.
    • Assists tenants in developing informal support networks with other tenants, family, and friends.
    • Screens tenants for eligibility and assists tenants with applications for appropriate benefits and entitlements.
    • Conducts referrals to JASA and other providers for the direct provision of those benefits including housekeeping, home health aides, public assistance, Medicare, Medicaid, SNAP, legal aid, protective services, vocational placement and volunteer opportunities, medical and psych exams, etc.
    • Medical care coordination and assistance including linking tenants to primary care providers, establishing transportation for appointments, and DME assistance.
    • Prepares specific information about the health, welfare, education, and recreational services available to serve the tenant including health literacy, substance use and treatment options, educational and support opportunities for management of chronic health issues.
    • Educates tenants on service availability, application and benefit procedures, tenant rights, and provides advocacy when appropriate.

Ensures tenants maintain housing subsidies and benefits required to remain in permanent housing.

  • Assists with tenant health screenings for blood pressure, depression, anxiety, memory, and substance abuse.
  • Completes in-home falls prevention/safety assessment to identify any fall risks or other unsafe situations.
  • Evaluates tenant functioning, monitors risk, assesses the status of current services, and updates and revises the case plan at ongoing in-home visits.
  • Provides regular reports to the Associate Director of Residential Services on common problematic themes/patterns of the tenants.
  • Consults with the Associate Director of Residential Services regarding high risk tenants including those tenants who may be at risk for eviction.
  • Performs other related tasks, special assignments, and duties as assigned.

Requirements:

  • Graduate of an Accredited College or University with a Master’s in Social Work, or other degree equivalent, preferably in sociology, gerontology, psychology, or human services.
  • Minimum two years relevant experience required.
  • Ability to work collaboratively within an interdisciplinary team.Computer proficiency including Microsoft Office Suite, Google Applications, and internet-based research.
  • Excellent interpersonal skills, including the ability to communicate effectively and concisely both in writing and orally.
  • Strong organizational and multitasking skills.
  • Bi-lingual English/Spanish preferred.
  • Ability to understand and appreciate the role of race, religion, ethnicity, sexual preference, and individual values as they relate to service clients and their families.
