Description

Job Summary:

Responsible for the timely coordination of the referral process including the review/response/coordination of care for patients referred to Jet Home Health. Responsible for the monitoring/review/ and response for patients primarily focusing on referrals and developing the referral relationships of the company within the community, in accordance with company policies and procedures. Interacts closely with facility representatives, discharge planners and other agencies to facilitate the initiation of care. Reverse marketing in coordination with the local Business Development Leader to reinforce messaging, stay top of mind and present information where it may be of value to our customers.

Requirements

Education: High School graduate or equivalent is a must. Graduate of an accredited school of Vocational/Practical nursing is preferred.

Licensure: Current state license as a Licensed Vocational/Practical Nurse or a multi-state license issued by a state member of the National Nurse Licensure Compact (NLC). Current drivers license.

Experience: One year experience as a Licensed Vocational/Practical Nurse, hospice experience preferred. General knowledge of spelling, punctuation, grammar, medical records and office procedures

Transportation: Reliable transportation and valid auto liability insurance.

Environmental and Working Conditions: Works in a home office environment. The ability to work a flexible schedule and extended hours. The ability to travel locally with some exposure to inclement weather. Possible exposure to toxic materials, toner, etc

Physical and Mental Effort: Prolonged sitting and some standing required. The occasional need to lift, pull, carry, and push items weighing up to 50 pounds. Frequent need to stoop, kneel, and reach while accessing files. Requires working under some stressful conditions to meet deadlines and patient needs, and to make quick decisions. Requires hand-eye coordination and manual dexterity. Requires excellent problem-solving skills

Essential Functions:

  • Assist with compiling patient information from referral source(s) including specific patient needs, patient demographics and insurance and/or payor sources in order to assure quality of communication, information and care for new patients. Receives and documents patient referrals to Home Health, by phone or in person for designated department, ensuring compliance with applicable coverage criteria.
  • Enter referral into Homecare Homebase
  • Participates in coordinating care with nursing supervisors, patients, and the interdisciplinary team.
  • Provides effective communication to patients, staff members, other healthcare professionals, and referral sources.
  • Serves as the first line of customer service for external and internal customers (Branch relationship).
  • Provide explanation of the benefits to patients, families, and community contacts
  • Responds to the Agency and patient needs in a professional and creative manner.
  • Maintain accurate records on all referral sources, status of existing or pending referrals, admissions, discharges, hospitalized patients; follow-up when needed.
  • Performs reverse marketing in coordination with the local BD Leader to reinforce messaging, stay top of mind and present information where it may be of value to our customers.
  • Meets assigned key performance indicators (KPIs).
  • Communicates pertinent patient information to the Branch team via approved processes.
  • Coordinates with other departments for insurance verification and payer authorization as needed.
  • Professionally communicates and follows up with referral sources regarding required documentation for a complete referral.
  • Knowledgeable in all Home Health processes
  • Gather appropriate information to assist the clinical team in scheduling an evaluation visit.
  • Makes referrals to other community resources as directed by the.
  • Communicates and follows up with Business Development regarding pending admissions or as directed by Intake Manager
  • Demonstrates commitment, professional growth, and competency.
  • Participates in the department sponsored in-service trainings.
  • Participates in the department QAPI program.
  • Adhere to work schedule and maintain good punctuality and attendance record
  • Provide telephone and clerical support to other staff members as time allows
  • Maintains accurate clinical records, keeping track of goals and progress of clients’
  • Abides by all regulations, policies, procedures and standards
  • Performs other related duties as assigned or requested.
  • Coordinates the evaluation/admission process.

Job Requirements:

  • Complies with policies, procedures and regulatory mandates including but not limited to abiding to the terms of the Jet Home Health Compliance Program 
  • Maintain proficiency with electronic medical records system
  • Reliable internet service and ability to office from home with minimal disruption to the workday
  • Attend required meetings and in-services
  • Ensure protection of confidential information that is stored, handled, or maintained by following company policies and procedures related to confidentiality and/or HIPPA regulations
  • Maintains positive internal and external customer service relationships
  • Plans and organizes work effectively and ensures its completion
  • Meets productivity expectations with quality documentation of complete referrals including all required follow up.
  • Demonstrates team behavior and promotes a team-oriented environment
  • Represents the organization professionally at all times
