Position Title: Life Enrichment Coordinator

Department: Life Enrichment

Supervisor: Life Enrichment Supervisor

Position Information: Full Time

Qualifications:

High School graduate or GED equivalent; ADC or ADC eligible. Basic understanding of the needs of the elderly and a willingness to work with a variety of people. Creativity and a basic knowledge of a variety of activities and arts and crafts. Must have a spirit of cooperation and a pleasant, friendly personality.

Essential Job Functions:

1.  Plan and lead group activities for residents.
2.  Plan and follow through with one-to-one activities for residents.
3.  Evaluate and assess residents’ needs in the area of therapeutic recreation and document findings on designated forms including the MDS 3.0.
4.  Identify problems and needs and develop a comprehensive care plan with the interdisciplinary team. Address social and recreational needs of the resident, set attainable goals and coordinate interventions.
5.  Evaluate resident's response to activities and interventions, communicate observations to care plan team and alter the care plan appropriately.
6.  Document residents’ progress and other observations on quarterly progress notes.
7.  Attend interdisciplinary care conferences as assigned.
8.  Complete assessments, care plans and other documentation on a timely basis, according to facility policy.
9.  Plan and create a monthly calendar for facility activities.
10.  Transport residents to and from activities.
11.  Set up and prepare areas for special activities and restore the area to its original condition at the conclusion of the activity.
12.  Work flexible hours as assigned in order to provide activities.
13.  Maintain an on-going inventory of departmental supplies and equipment.
14.  Plan and organize his or her day's work in a time and cost-effective manner.
15.  Set priorities for the day's tasks but is also able to remain flexible to meet the needs of the department and the residents.
16.  Work independently and to function as an effective team member.
17.  Attend in-service training and continuing education workshops as assigned. Integrates content of continuing education into performance.
18. Interpret verbal, physical, or social responses from residents, whether appropriate or not, to decipher resident needs. 
19. Understand and comply with the corporate compliance program of Jennings Center, Garfield Heights.
20. Look beyond typical “activity” duties to benefit overall care.
21. Coordinate the use of the facility’s van. This includes finding a driver, ensuring the gas tank is full, leaving the van clean and tidy, and reporting any problems to maintenance.
22. Notify nursing personnel if a change in resident's condition is noted.
23. Provide a safe environment for residents, staff, volunteers, and visitors.
24. Assign tasks to volunteers and monitor their performance.
25. Perform other duties as directed by the Activity Director.
26. Observe infection control procedures.
27. Follow Resident's Rights policies and procedures and assist the residents in understanding and maintaining their rights
28. Observe all facility safety policies and procedures.
29. Come to work as scheduled and consistently demonstrate dependability and punctuality.
30. Consistently work cooperatively with all staff, residents, family and visitors.
31. Accept assigned duties in a cooperative manner.
32. Assume accountability for data contained in the staff handbook.
33. Focus on person-centered care by promoting day to day interactions that are essential to that philosophy.

a. Forming personal and nurturing relationships with the Residents.

b. Taking the initiative to find out what is important to the Residents and ways in which to connect them with the things that they need to combat boredom, loneliness and helplessness.

c. Utilizing person -centered care to create interactions and execute those interactions either individually or as part of a group.
