Job Summary:
We are seeking a skilled and compassionate Family Nurse Practitioner to join our healthcare team. As a Family Nurse Practitioner for Jemericus, you will provide comprehensive primary care to patients that currently reside in Adult Living Facilities. Your role will involve diagnosing and treating acute and chronic illnesses, promoting health and wellness, and collaborating with other healthcare professionals to ensure the highest quality of patient care. You will need reliable transportation.

Duties:
- Conduct thorough patient assessments, including medical history, physical examinations, and diagnostic tests
- Diagnose and treat common acute and chronic conditions, such as respiratory infections, diabetes, hypertension, and musculoskeletal disorders
- Prescribe medications and develop treatment plans based on evidence-based guidelines
- Provide counseling on disease prevention, healthy lifestyle choices, and management of chronic conditions
- Collaborate with specialists to coordinate patient care
- Maintain accurate and up-to-date medical records
- Educate patients and their families about their healthcare needs and treatment options

Requirements:
- Valid certification as a Family Nurse Practitioner
- Current state licensure as an Advanced Practice Registered Nurse (APRN)
- Master's degree in Nursing (MSN) or Doctor of Nursing Practice (DNP)
- Strong knowledge of human physiology and pathophysiology
- Experience with Adult Living Facilities is preferred
- Familiarity with Athena electronic medical record system
- Excellent communication skills to effectively interact with patients, families, and healthcare team members
- Ability to work independently as well as collaboratively in a multidisciplinary setting

We offer two employment routes, one is salaried where you will receive sick time and a generous vacation package. We also offer percentage based salary which can be between 65 and 80 based on experience. We are very willing to negotiate on that end.

Pay: $105,000.00 - $130,000.00 per year
