Jefferson Day Report Center is Hiring a Public Safety Case Manager Near Kearneysville, WV

We are looking for a Public Safety Case Manager to enhance the quality of client management for the Driver's Safety and Treatment Program and maximize public safety by promptly enrolling and managing high-risk populations into the program. The successful candidate will be accountable for the full client care cycle. Public Safety Case Manager duties include assessing, planning, implementing, monitoring and evaluating actions required to meet the client’s psychoeducational programming needed along with promoting public safety by ensuring clients successfully complete the programing required through the West Virginia DMV.

Responsibilities

  • Coordinate and provide care and psychoeducational treatment that is safe, timely, effective, efficient, equitable, and client-centered.
  • Handle case assignments, draft service plans, review case progress and determine case closure.
  • Help clients achieve wellness, reocovery, and autonomy.
  • Facilitate multiple care aspects (case coordination, information sharing, etc.)
  • Help clients make informed decisions by acting as their advocate regarding their driving and clinical status/treatment options.
  • Develop effective working relations and cooperate with the mental health and/or medical team throughout the entire case management process.
  • Arrange abeyances with social services, health and governmental agencies.
  • Promote success with weekly status check-ins and interact with clients to keep track of their progress and to ensure satisfaction and program completion.
  • Record cases information, complete accurately all necessary forms and produce statistical reports.
  • Submit all program completion records to the Executive Director and the WV DMV as required by law.
  • Promote quality and cost-effective interventions and outcomes.
  • Assess and address motivational and psychosocial issues with referrals for mental health assessment.
  • Adhere to professional standards as outlined by protocols, rules and regulations.
  • Document and chart group education sessions clearly and concisely;
  • Interact with staff and participants, providing guidance and assistance as per program rules and procedures;
  • Maintain currency in best practices and knowledge of addiction treatment strategies;
  • Provide service, education, and advocacy for recovery;
  • Complete Addiction Severity Index assessments;
  • Work closely with the treatment staff, reporting concerns or suggestions regarding participant’s risks and needs on an as-needed basis;
  • Performs general office duties that include organization of files, answering phones, use of fax machine and copy machine, typing, sending emails, etc.;
  • Other duties as required and is subject to change at any time

Skills

  • Proven working experience in public safety, case management, or related field.
  • Previous experience with psychological aspects of care
  • Effective communication skills
  • Excellent organization and time management skills
  • Familiarity with professional and technical emerging knowledge
  • Problem solving skills and ability to multi-task
  • Compassionate with teamwork skills
  • Current drivers license.
  • BS degree in related public safety or human services field.

Job Type: Full-time

Pay: $20.00 - $25.00 per hour

Expected hours: 40 per week
