RESPONSIBILITIES:
Job Specific Competencies:
Establish primary care for new and transferring patients in a timely manner, including answering questions regarding providers, and clinic services; via telephone, in person, or by JCMG email/website, and ECW/referral messages.
Call patients to reestablish JCMG care after a physician departure and help manage patient communication.
Promote positive public relations by maintaining a professional appearance and demeanor, demonstrating appropriate email and phone etiquette.
Coordinate transfer of medical records as necessary.
Check PCP schedules daily/weekly to observe and manage empty times to be filled.
Facilitate translation services as needed.
Facilitate timely and professional communication between providers and clinic staff.
Maintain reports to senior leadership and or departments of patient placement into physician practices, insurance coverage, and patients not placed with a primary care provider
Reports to manager on a monthly basis, or more often as requested.
Attend health fairs and other appropriate community relations and outreach events to promote JCMG as needed.

MINIMUM QUALIFICATIONS
Education:
Equivalent to a high school education
Experience:
Six months to two years of similar or related medical experience
Certification/License:
NA
Knowledge/Skills/Abilities:
Must be a self-starter and able to communicate with all staff, physician groups and patients in a professional manner. Obtaining cooperation (internally and externally) is an important part of this position and requires a level of diplomacy and trust. Independent judgment is often necessary and the ability to efficiently handle conflict and negotiations is crucial to success in this role
