Solid Ground Apartments (SGA) is a permanent supportive housing program committed to providing stable and dignified housing solutions for individuals facing homelessness. Our organization believes in fostering a supportive and empowering environment to help residents regain control of their lives. Solid Ground Apartments offers a comprehensive range of services aimed at addressing the unique needs of each resident, promoting self-sufficiency, and facilitating a successful transition to permanent housing. SGA residents are identified to be currently experiencing or at risk of chronic homelessness, high utilizers of publicly funded systems, possibly disengaged from services and in need of diverse supportive services.

Solid Ground Apartments is seeking a dedicated and compassionate Case Manager to join our team. The Case Manager will play a crucial role in assisting residents in achieving stability and accessing necessary resources. This position requires a proactive, client-centered approach to address the complex challenges faced by individuals experiencing homelessness. This role is tasked with identifying all types of barriers which may complicate a participant’s ability to maintain housing and use creative problem solving to address these issues. This position includes broad case review, scheduling and outreach, and various other supports to monitor client progress and ensure that SGA outcome measures are met. This position will be located at 7272 W 14th Ave. Lakewood, CO 80214.

Essential Duties:
  • Provide case management to SGA residents, with an avg. caseload of 20 clients.
  • Conduct thorough assessments to identify individual needs, strengths, and barriers to stability.
  • Collaborate with residents to develop realistic and achievable goals.
  • Establish and maintain collaborative relationships with residents, service providers, and community resources.
  • Advocate for residents in accessing necessary services, including healthcare, mental health, employment, and education. Actively coordinate care with Jefferson Center, and other community agencies and health providers.
  • Respond promptly and effectively to crises, providing emotional support and connecting residents with appropriate resources. Collaborate with the crisis intervention team to ensure a coordinated and trauma-informed response.
  • Responsible for the coordination and/or completion of needed paperwork including releases of information, HMIS intake and assessment, Homeless Prevention Demographics Profiles, and other needed documentation.
  • Works from a strength based, recovery-based perspective and takes cultural considerations into account when appropriate.
  • Document all contacts according to Jefferson Center standards while meeting productivity standards as identified by the Program Director.

Other Duties:

  • Submit 90% of all SFRs/Progress-to-Date forms within 3 working days.
  • Exhibit enthusiasm, courtesy, adaptability, flexibility, and spirit of cooperation in the work environment.
  • Maintain effective interpersonal relations and customer service with consumers, peers, upper management, visitors and the general public.
  • Ability to multi-task, prioritize concerns, and effectively manage time independently.
  • Effectively respond to client/consumer needs and problems, initiate and maintain positive recovery-based interactions, timely response to phone calls, texts, emails and other requests.
  • Participate in team meetings and clinical consultations as scheduled.
  • Participate in supervision by coming prepared with an agenda. Report high risk/problem cases and utilize a problem-solving approach as well as feedback.
  • Attend supervision at times and intervals agreed upon with supervisor.
  • Other duties as assigned by the Director of Navigation and Housing Services.

Minimum Education, Knowledge, Skills & Experience Required:
  • Bachelor’s Degree in psychology or related field – required.
  • Two or more years of experience in case management, social services, or supportive housing preferred.
  • Strong understanding of issues related to homelessness, mental health, and substance use.
  • Experience with Housing First and Harm Reduction models preferred, knowledge of Fair Housing a plus.
  • Good communication, organizational, and computer skills.
  • Ability to work collaboratively in a team and independently.
  • Consistent evidence of timely and accurate work products required.

Salary Grade 50- Bachelor’s Degree (Case Manager) $39,100 to $49,800* (18.80/hr - $23.94/hr)*Additional 10% added to base salary for Housing.

Additional Salary Information*:

  • All salaries at Jefferson Center are calculated based on years of total relevant experience.*
  • The salary range above is based on 1.0 FTE (full time equivalent) or 40 hours per week. Less than 40 hours/week will be prorated and adjusted to the appropriate FTE.*
  • Additional pay provided for evening, overnight, and weekend shifts.*

*This position qualifies for a limited time grant funded bonus of $1000 paid on the one month anniversary!* 

Additional Bonus Information*:

  • Based on full time (40 hours/week). Prorate if less than 40 hours/week* 
  • Must work a minimum of 20 hours/week to be eligible for bonus* 
  • Current Jefferson Center employees not eligible for sign on bonus*
