JOB DESCRIPTION: DHR Unit Low Intensive Care Coordinator

WORKING TITLE: Care Coordinator

This Care Coordinator will be serving the JBS Mental Health/JCDHR Mental Health Unit. The Care Coordinator will provide assessment for children and adolescents with symptoms of severe emotional disturbance and behavioral problems that are interfering with their functioning in any life domain as referred through the JBS/JCDHR Unit. The Care Coordinator will provide coordination of services, crisis intervention and family support as indicated by the assessment. The Care Coordinator will provide referrals for clients in need of services beyond the treatment available through the JBS/JCDHR Unit and will provide support to clients, parents and collateral contacts to link patients to appropriate resources for care.

Supervised By: Children's Clinical Director

Division: Children's Services

Normal Working Hours/Days: Monday-Friday 8:00 a.m.-4:30 p.m. with flexibility to meet client/family need.


Primary Job Functions

Provide access to a wide range of services for children/adolescents with a serious emotional disturbance and their families as assigned by the supervisor

Responsible for providing assessment on a DMH approved assessment tool to identify the unmet needs of the target population and devising an individualized care plan for each client.

Collaborates closely with the DHR staff, as well as, other community resources in order to provide or access appropriate services to meet the needs of the target population (mental health services, school/special education service, etc.)

Serve as a liaison between the client and various service providers to ensure continuity of care.

Provides accurate documentation of charts on a daily basis, including assessments, care plans, referrals, data collections, case reviews and follow up tracking in an electronic medical record

Responsible for providing and presenting assessments information and goals at weekly staffings with supervisor.

Effectively implement written care plans to meet client goals and objectives.

Provide transportation to clients when necessary.

Performs other administrative duties as required.


Minimum Qualifications

Bachelor’s Degree in psychology, counseling, social work or behavioral sciences related field from an accredited university required.

Valid Alabama driver's license and driving record acceptable to agency automobile insurance carrier for inclusion on agency policy. Suitable automobile to permit the required travel. Must show and maintain car insurance equal to or in excess of the State of Alabama standards for automobile insurance coverage.


Knowledge, Skills and Abilities

Knowledge of the special needs and behavioral characteristics of children/adolescents with a serious emotional disturbance and their families.

Knowledge of community and available community resources

Knowledge of rules and regulations governing the human services provider agencies in the catchment area.

Knowledge of the legal and ethical issues relative to confidentiality of client records and the ability to interpret and apply them within the scope of the care coordination responsibilities.

Knowledge of casework principles and methods related to care coordination.

Ability to communicate effectively with a multi-disciplinary team to include Department of Public Health, Department of Human Resources, Juvenile Probation Office/Family Court, School Staff, and other mental health professionals.

Able to navigate and work with-in an electronic medical record.

Demonstrate communication skills - both expressive and written.

Ability to organize and manage one’s own activities with a minimum of direct supervision.

Ability to work flexible hours due to the special needs of the target population and their families.
