Ivy Hill Post Acute is an exceptional Team-oriented Company, seeking a Director of Social Services! We Pride ourselves on our caring and compassionate management team who are there to fully support our staff and residents.

The Ideal candidate will have a Degree in Social Work with prior experience in long-term care.

Candidates should possess strong discharge planning skills, excellent organizational and communication skills, ability to handle multiple priorities efficiently, strong customer services skills. Additionally, strong applicants will possess the ability to work collaboratively across a variety of disciplines.

Responsibilities:

  • Ensure the resident(s) have a smooth transition into the facility
  • Complete comprehensive psychosocial assessments for each resident’s social, emotional, and psychological needs • Identifies social needs of residents, and provides appropriate services to meet the individual’s
  • Collects and compiles resident social information into personal file
  • Maintains lists of facility and community resources available to the residents that can provide assistance in the community
  • Plans, organizes and implements Social Service programs in the facility
  • Evaluates effectiveness of Social Service programs in the facility and collaborates the Social Services team, Administration and other facility staff to offer best practices to the residents
  • Assesses each resident for discharge and coordinates discharges in the facility
  • Advocates for resident’s best interest and total care in Social Services and clinical care
  • Conducts resident interviews and ensures that relevant MDS sections (i.e. cognitive, mood, behavior, patient goal setting) and Care Area Assessments are completed in accordance with regulation
  • Complete monthly reports as required
  • Performs other related job duties as required

Social Worker Qualifications: • Bachelor’s Degree

  • Long Term Care experience

Job Type: Full-time

Pay: $56,194.95 - $81,597.00 per year
