IV Care of Middle GA is Hiring a Patient Care Manager (Nurse Manager) Near Milledgeville, GA

The most interesting and exciting things you get to do in this position is: serving patients and families with an interdisciplinary approach, serving your community, building rapport with other healthcare providers, educating, helping your team through dynamic situations, and serving people at one of the most critical and sacred moments in life. We are looking for a Registered Nurse who will be responsible for the implementation, delivery, coordination, organization, and supervision of the hospice team. Also, this individual coordinates scheduling of staff, monitors the performance of staff, assists in the orientation of staff, and serves on committees to improve operations. He/she is responsible for the provision of skilled assessments, reassessments, development/implementation/revision of the plan of care, and delivery of care in order to maximize the comfort and health of patients and families consistent with agency policies and procedures.

EDUCATIONAL REQUIREMENTS

  • Possess a current license to practice as a registered professional nurse in the state of Georgia
  • Graduate of an accredited school of nursing.

EXPERIENCE REQUIREMENTS

  • Minimum of two (2) years experience as a Registered Nurse and preferably one of the two years in an acute, oncology home health, and/or hospice setting.
  • Knowledge of the Medicare Certified Hospice Program preferred.

OTHER REQUIREMENTS

  • Possess a valid driver’s license in the state of Georgia.
  • Ability to either drive or have available reliable transportation and present evidence of valid automobile liability insurance.
  • Ability to communicate effectively, orally and in writing, and to comprehend written and verbal communication.
  • Ability to operate computer equipment, fax, and copier machines.
  • Ability to work a flexible schedule.
  • Ability to work well within an interdisciplinary team setting as well as independently.
  • Ability to work with patients and families dealing with life-threatening illness and death.
  • Ability to assess and respond to the needs of patients and families in varied settings.
  • Demonstrates skill in nursing practice, ability to cope with family’s emotional stress, and tolerance of individual lifestyles.
  • Ability to make meaningful observations and write comprehensive accurate reports.
  • Demonstrates excellence in managing human resources.
  • Demonstrates effective public speaking skills and the ability to develop good interpersonal relationships.
