IRMC is Hiring a Social Worker/Clinical Care Coordinator - IRMC Physician Group - Full Time Near Indiana, PA

Qualifications

  • Must have met the requirements of a master’s curriculum in a school of social work from a Council on Social Work accredited (CSWE) or actively pursuing MSW (CSWE) program or a master's equivalent.
  • Computer applications related to the work including proficiency in Microsoft Word and Excel.
  • Ability to perform techniques such as: motivational interviewing, brief intervention, and psychosocial assessment.
  • Knowledge of public and private social services providers within the community and programs available to clients with identified needs.
  • Ability to engage with a variety of individuals from various socio-economic, ethnic and cultural backgrounds, in person and over the telephone.
  • Understanding concepts of healthcare continuum, case management, and population health. 

Responsibilities:

  • Engage in ongoing program development, provide input, ideas, and feedback to team on system function, efficacy and efficiency.
  • Participate in continuous quality improvement and training opportunities.
  • Assisting patients with access to healthcare and navigating the healthcare continuum.
  • Care coordination & case management for high-risk patients follow up post discharge.
  • Supporting provider interventions for patients with chronic conditions.
  • Utilizes a team approach, confers with physician and ancillary staff to provide quality care to patients.
  • Use initiative and independent judgment; organize own work, set priorities, and meet critical deadlines.
  • Complete ongoing documentation of daily interactions including assessments, treatment planning and documentation, while maintaining confidentiality and privacy mandates.
  • Assist & participate in community outreach, support groups, committees, and evening programs as needed.
  • Services by phone and/or face-to-face service; tracks client progress and well-being, evaluates treatment effectiveness, while making adjustments based on need.
  • Collect, analyze & interpret data relevant to oncology needs. Dissemination of reports.
