Iowa Care Community Inc is Hiring a Clinical Nurse Coordinator (Hospice) Near Iowa, IA

Description

Iowa City Hospice is seeking a highly motivated and compassionate Clinical Nurse Coordinator to join our hospice team. This position is a vital member of our interdisciplinary team and serves as a resource through assessing and managing complex and challenging situations. Most days are spent coordinating care and assisting field staff to ensure care is efficient, effective, high quality and in regulatory compliance. The ideal candidate will have strong clinical skills, excellent communication abilities, and a passion for our Mission; to provide compassionate care for anyone in our community affected by serious advanced illness and end of life conditions.

Responsibilities:

  • Serve as a resource and advocate for patients and their families, providing education, emotional support, and guidance throughout the hospice journey.
  • Facilitate communication and collaboration between healthcare team members to ensure continuity of care and optimal patient outcomes.
  • Participate in interdisciplinary team meetings, case conferences, and quality improvement initiatives to enhance patient care and program effectiveness.Coordinate and manage the care of hospice patients in collaboration with the interdisciplinary team, including physicians, nurses, social workers, and other healthcare professionals.
  • Triage comprehensive assessments of patients' physical, emotional, and spiritual needs while reviewing individualized care plans and making recommendations to staff.
  • Maintain accurate and timely documentation of patient assessments, care plans, and interventions in compliance with regulatory standards and organizational policies.
  • Ensure compliance with all regulatory requirements and standards of care

Requirements

  • Graduate of an approved school of nursing (RN/BSN/ARNP) and current Iowa Registered Nursing License.
  • Bachelor's degree in Nursing (BSN) preferred
  • Minimum of two years of clinical nursing experience, preferably in hospice, palliative care, or oncology
  • Certification in hospice and palliative care nursing (CHPN) preferred
  • Compassionate and empathetic approach to patient care
  • Ability to work effectively in a fast-paced and emotionally challenging environment
  • Strong clinical knowledge, critical thinking skills, and evidence-based practice expertise.
  • Excellent communication, interpersonal, and organizational skills.
  • Ability to prioritize tasks, delegate responsibilities, and manage multiple priorities effectively.
  • Knowledge of healthcare regulations, accreditation standards, and quality improvement methodologies.
  • Proficiency in computer skills, including electronic health records (EHR) and Microsoft Office applications.
  • Demonstrates leadership and decision making abilities with knowledge and effective collaborative interpersonal skills.
  • Adhering to ethics, standards, and the support of agency policies and procedures.
  • Car, driver's license and adequate insurance as outlined in the personnel policies is required.
  • All offers of employment are conditioned upon the result of a criminal background check conducted by the Iowa Division of Criminal Investigation, as required by law and a negative finding on the Office of Inspector General’s Excluded Provider List.
  • Additional Training: Advanced Certified Hospice and Palliative Nurse (ACHPN®) certification in hospice and palliative care, by the 3rd year of employment.Active Registered Nurse (RN) license in Iowa
  • Available to work every shift; Monday through Friday, 8 AM – 5 PM; includes participating in holiday and services on call rotations.
