We are looking for a compassionate Certified Home Health Aide/In-Home Caregiver to provide one on one assistance with daily living and personal care services in patients homes in accordance with an established care plan.

Job Summary:

• Provides personal care according to a written plan of care in patients residence
• Monitor patients (vital signs, temperature, respiration, etc)
• Reports patient status and significant changes regularly to the Case Manager
• Assists with light housekeeping, meal preparation and other duties as outlined in the plan of care.
• Maintain patients care records and document provided services
• Assist patients with mobility and physical therapies/exercises
• Medication reminders assistance
• Provide companionship and basic emotional or psychological support

Skills

• Proven working experience as home health aide and In-home caregiver
• Caring and compassionate personality
• Ability to monitor vital signs
• Proficiency in English
• Knowledge of other languages is a PLUS
• Current CPR certificate preferred
• First Aid training preferred
• Valid driver’s license
• Reliable transportation
• High school degree

Job Types: Full-time, Part-time

Pay: From $15.50 per hour
