Registered Nurse (RN) Case Manager - Home Care: Enjoy independence and flexibility in the field to care for patients in their home environment.

A few must-haves for this position:

  • Graduate of an accredited school of nursing and has a current license as a Registered Nurse in the State of MN.
  • Able to travel within a geographic area serviced by the office. Reliable transportation, valid driver's license, and proof of auto insurance required.
  • CPR Certification
  • Two (2) years of RN experience.
  • One (1) year case management experience preferred.
  • Community health and a wide range of Medical-Surgical experience preferred.

Some functions of what you would do as an RN Case Manager:

  • Provides services that are ordered by the physician as indicated in the plan of care.
  • Completes nursing initial and ongoing assessments of all in home clients within specified deadlines.
  • Participates in ongoing interdisciplinary assessment of the patient.
  • Participates in the development and evaluation of the plan of care in partnership with the patient, representative (if any), and caregiver(s).
  • Visits in home clients to evaluate the quality of care being provided and review clinical notes.
