Job Summary:
We are currently seeking a highly organized and compassionate individual to join our team as a Care Coordinator at Interim HealthCare - Montgomery County & Southern MD. This individual will be responsible for coordinating care services for our clients in the Gaithersburg, Maryland area. The Care Coordinator will play a crucial role in ensuring that our clients receive quality care services and will work closely with both our clients and caregivers to facilitate a positive and supportive care experience.
Compensation & Benefits:
This is a full-time, salaried position with a compensation range of $50,000 to $55,000 per year. We also offer a comprehensive benefits package that includes medical, dental, and vision insurance, paid time off, and a retirement savings plan.
Responsibilities:
- Serve as the main point of contact for our clients, caregivers, and healthcare professionals involved in the care process.
- Coordinate with our team to develop and implement individualized care plans for each client.
- Act as an advocate for our clients, ensuring that their needs and preferences are met.
- Conduct intake assessments and regularly evaluate and reassess client care needs.
- Schedule and coordinate client appointments and communicate any changes to the appropriate parties.
- Ensure proper documentation and records are maintained in compliance with company policies and procedures.
- Collaborate with caregivers, clients, and healthcare professionals to address any concerns or issues that may arise.
- Provide support and resources to clients and caregivers to promote successful care outcomes.
- Participate in team meetings and provide guidance and feedback to improve care processes.
Requirements:
- Bachelor's degree in healthcare administration, social work, or related field.
- Minimum of 2 years experience in care coordination or a related role.
- Knowledge of healthcare regulations and best practices.
- Excellent communication and interpersonal skills.
- Strong organizational and time-management abilities.
- Proficiency in Microsoft Office and electronic medical records systems.
- Compassionate and patient-focused mindset.
